Default How To Build a Survey on our website

I am looking to build a survey on our website that I could ahve the completed information forwarded by email to me. Is there a way?

Hi,

If you have design a survey which would generally involve a form,buttons with
actions (may be could be sending a mail with all information) - This can be
done by dreamweaver. Contribute mainly focusses on editing the website content.
